Application for Membership

Below is a web link to an electronic application for membership in the MVRS, Inc. Please fill it out completely and provide any copies of certifications you may already hold (CPR, EMT, etc.).

The Health History Form is also below and must be completed by you, signed by your physician and provided to our secretary prior to you being turned over to ride the ambulance.

Please also fill out and sign the Consent Release Form for the criminal background check.

For clarification purposes:

• An Active Member is someone who is or wants to be an EMT or higher and who has turned 18 years old.
• A Driver-Only Member is someone who is at least 21 years old and who wishes to only join as a driver (CPR required).
• An Associate Member is someone who is a member in good standings at another fire/rescue department in the tri-county area (Calvert, Charles. or St. Mary's) with EMR or higher.  
• A Provisional Member is a person who is at least 16 and wishes to become an EMT but has not yet turned 18; or who is an Emergency Medical Responder (EMR).
• A Cadet member is someone interested in membership who is 14 or 15 years old.
• An Administrative Member is one who does not wish to drive or become certified as an EMS provider and will only perform administrative duties as assigned. These members do not ride on the ambulance.

Once you have turned in your application our President and the Chief will meet with you to introduce themselves and tell you about our department.

Our Business Meetings are held on the 3rd Wednesday of every month at 7PM. Your application will be presented to the membership at the next business meeting after you have completed the above. Once your application has been processed at our meeting you will be contacted by our Secretary. After that meeting, you will be given an Orientation presentation to familiarize you with our building, apparatus, by-laws and Standard Operating Guidelines.
